Organoprotective Effect of Remote Ischemic Preconditioning during Living Donor Liver Transplantation, A Prospective Randomized Double-Blinded Study

چکیده
Background: Ischemia reperfusion injury (IRI) during liver transplantation carries a substantial risk for graft damage, and other major organ injury. Strategies to minimize IRI in the grafted are of paramount importance. Remote ischemic preconditioning (RIP) is recently described technique that can offer protective effect against IRI. Patients Methods: In this prospective randomized study, consent was obtained from 80 couples (donor– recipient) prepared (LT). were divided randomly (using closed envelope technique) into two groups: RIP group (n=35) as procedure run donor non-dominant hand just at start parenchymal transection control (n= 35). all study cases, pressure cuff wrapped around recipient arm. group, four sets 5 minutes manual elevation 200 mmHg, separated by release (Using hand). Results: No statistically significant difference found between biochemical profile (SGPT, CRP, bilirubin) recipients both groups. Similarly, post-operative kidney function (Serum creatinine) comparable findings studied Conclusion: living failed show neither on response injury, nor postoperative functions. Future studies recommended taking consideration histopathological, tissue damage scores.

Ischemic conditioning involves the delivery of short cycles of reversible ischemic injury in order to induce protection against subsequent more prolonged ischemia. متن کاملLiving Donor Liver Transplantation
Living donor liver transplantation (LDLT) is probably the most high-profile of all surgical enterprises. At the same time, it is an amazing act of altruism. It requires hard work of dedicated multidisciplinary medical teams coupled with the courage of the patients and their families. The concept of LDLT is based on the following two factors: (1) the remarkable regenerative capacity of the liver...
متن کاملRemote Ischemic Preconditioning in Children Undergoing Cardiac Surgery With Cardiopulmonary Bypass: A Single‐Center Double‐Blinded Randomized Trial
BACKGROUND Remote ischemic preconditioning (RIPC) harnesses an innate defensive mechanism that protects against inflammatory activation and ischemia-reperfusion injury, known sequelae of cardiac surgery with cardiopulmonary bypass. We sought to determine the impact of RIPC on clinical outcomes and physiological markers related to ischemia-reperfusion injury and inflammatory activation after car...
